Surgery, Scheduling and Procedures, Forms


Scheduling
Your physician's office will call us to schedule a date and time for your surgery or procedure.  You will be provided a "Patient Handbook" which describes our facility, a "Patient Data Form", a "Health Questionnaire", "Medication Reconciliation" and a copy of your patient "Bill of Rights".  We ask that you complete these forms to the best of your ability in order to ensure the most optimal surgical experience.  The forms can be downloaded from this website for your convenience.  Please mail (if time permits), fax or bring these forms the day of your procedure.

Before Your Surgery or Procedure
A nurse from the Naugatuck Valley Surgical Center will call you on the day before your surgery or procedure between the hours of 7:00am and 3:00pm to confirm your appointment, give you your scheduled time, complete a pre-operative questionnaire and answer any questions that you may have. If you are going to be unreachable during the day between those hours, please call the Center at (203)755-6663 and ask to speak to a pre-op nurse.

On the Day of Your Surgery or Procedure
You will be greeted by one of our Admitting Associates and they will obtain and verify your insurance information. When this is completed, a nurse will escort you to your room in PACU or GAPU, do a medical interview, and assist you in getting ready for your procedure. You will also meet and be interviewed by the anesthesiologist if you are going to receive sedation or anesthesia. A Registered Nurse, who will be assisting your doctor, will also come to meet you and escort you to the operating room or procedure room. When your procedure is completed, you will return to the post anesthesia recovery area, either in the PACU (Post Anesthesia Care Unit) or GAPU (Gastro and Pain Unit) and be cared for by an all RN staff until you are recovered.

After Your Surgery or Procedure
You will stay in our PACU or GAPU until it is determined by the anesthesiologist or your doctor that you are ready for discharge. At this time, your nurse will review your post operative instructions with you and the person that you have designated as a responsible adult. You will also be given written post operative instructions. It is very important that you have a responsible adult drive you home from the surgical center. After receiving any type of sedation, it is required that you not drive until the next morning. This is for your safety. As you know, medications last in your blood stream longer than what you can actually feel. It is because of the “half life” of many medications that we require that you do not drive on the day of surgery. Our anesthesia department will cancel your surgery or procedure if you are unable to accommodate this requirement. If you have any questions about this, please feel free to call us at (203)755-6663.

Day After Your Surgery or Procedure
One of our nurses will call you on the day after your surgery or procedure. If you are not going to be at one of the numbers that you previously provided to us, please inform your nurse of the number where we can contact you.

If the Patient is a Child, Please Remember:
At least, one parent or guardian must remain at the Naugatuck Valley Surgical Center while the child is at our facility. A parent or guardian must sign all consents for surgery if the child is under 18 years of age or unable to sign for any reason. Guardians must bring written proof of guardianship on the day of surgery.

 

Forms

 We have made our new patient forms available to you for your convenience.  To best care for your medical needs, we recommend printing and completing these forms at home. Please mail (if time permits), fax or bring these forms the day of your procedure. Having the ability to evaluate your medical history and process your demographic information prior to your procedure will help ensure the most optimal surgical experience.   If you have any problems printing these forms properly, please contact our facility.  We will gladly mail or fax them to you.
