Billing
Naugatuck Valley Surgical Center will bill your insurance company with the information that you provide to us for the services rendered at the time of your visit. The Surgical Center participates with most local and national insurers, HMOs, PPOs, IPAs and Workers Compensation carriers. We also participate with Medicare and most Medicaid programs.
If you have any questions as to whether the Center participates with your insurance carrier, please contact our billing department or your insurance carrier. You are responsible for any deductibles, co-insurance or One Day surgery co-pays. Co-pays are due at the time of your visit.
You will receive a bill from the Naugatuck Valley Surgical Center for any balance that is billable to you after your insurance has paid. You will also receive a separate bill from your surgeon, anesthesiologist, pathology and/or radiology departments depending on which services were utilized when you were a patient at our center.
Non-Covered Procedures or Patient Without Insurance
Patients having non-covered or cosmetic procedures, or uninsured patients, are asked to settle their account before surgery is performed. Our Billing Associate will contact you with an estimate of charges for the procedure that your surgeon has scheduled for you. All charges are estimated and may differ from the estimate if additional or different procedures are performed. The patient is responsible for any additional charges that result. Any unpaid balance will be billed to you and payment is requested within thirty days of receipt of statement.
Workers Compensation
Worker's Compensation claims need additional information. Please make sure that you provide the following information on the "Patient Data" form:
• Worker's Compensation Insurance Carrier name and contact person
• Employer name and address
• Date and time of injury
• Claim number
